Serum Release to Another Institution

Occasionally, patients need to take the allergy immunotherapy serum that was formulated at Tidewater Allergy and Asthma, and receive their AIT injections at another institution.  This is common for college students, for example, who need to receive their injections at student health during the school year. We are happy to help patients with this request and have outlined the necessary steps below. Please don't hesitate to reach out with any questions.

Patients who want to take their serum to another facility to receive injections need to do all of the following:

 

  1. Read and Sign “Letter to Patient Requesting to Receive AIT Injections at Outside Facility” 

  2. Send the “Letter to Supervising Physician at Receiving Medical Facility” to the physician who will administer your injections. They will need to read the letter, then complete and sign the form and return it to us.

  3. Once our office has received the letter from your physician and has notified you that your serum is ready for pick up, you will need to come to our office for pick up. At that time, you will need to sign the “Serum Release Agreement” stating that you take responsibility for the serum and understand how it needs to be transported.

  4. When you pick up your serum, please make sure you leave our office with the following

    • Your serum

    • A copy of your AIT injection schedule that includes your most recent injection (if applicable)

    • If the facility where you will receive your injections requires us to fill out a specific injection schedule form, please be sure to let us know and make arrangements for completion.

  5. Always remember you can call us with questions, or send a message through the patient portal, at any time.

  6. When you come back for a follow-up, please bring a copy of your injection schedule showing all of the injections that your received at the other facility, along with notes of any reactions.

 

Note: If the patient does not already have serum at our office, they will also need to complete the process to have the serum mixed.  This requires:
